By submitting the information requested in this form and participating in our mystery flight survey, you consent to the collection, use, reproduction, hosting, transmission, and disclosure of any such information as we deem necessary for the provision, modification and/or improvement of our services. Personal information will not be shared outside of our organization without your further consent. 

Our organization may collect and compile Aggregated Statistics. “Aggregated Statistics” means data and information related to your participation in our mystery flight survey and information submitted in this form that is used by our organization in an aggregate and/or anonymized manner, including to compile statistical information related to the provision of our services. As between you and our organization, all right, title, and interest in Aggregated Statistics, and all intellectual property rights therein, belong to and are retained solely by our organization. You hereby acknowledge that our organization may compile Aggregated Statistics based on user data input into this form or otherwise provided in connection with participating in our mystery flight survey. You agree that our organization may (i) make Aggregated Statistics publicly available in compliance with applicable laws, and (ii) use Aggregated Statistics to the extent and in the manner not prohibited under applicable laws; provided that such Aggregated Statistics do not identify you or your confidential information.

You hereby irrevocably consent and grant permission to have your photograph and/or an audiovisual recording of you taken during your participation and involvement in our mystery flight survey (collectively, “Materials”) and you further irrevocably grant permission, in perpetuity to our organization to use the Materials, in connection with internal or external promotional, publicity, and editorial purposes, advertising and promotional materials and any other purpose that our organization may, in its sole discretion, deem proper. You hereby waive any right to review, inspect or approve such use or Materials. Further, you hereby irrevocably assign to our organization all right, title, and interest, including copyright you have (if any) in and to the Materials and hereby waive in favour of our organization any moral rights in and to such Materials.
